Linezolid inhibits bacterial protein synthesis by binding to 23S fraction of 50S ribosome and interferes with formation of the ternary N-formylmethionine-tRNA-70S complex. The binding of Linezolid distorts the tRNA site overlapping both 50S and 30S ribosomal subunits and stops protein synthesis before it starts.

The use of directly and indirectly acting sympathomimetic agents (e.g. pseudoephedrine), vasopressive agents (e.g. epinephrine, norepinephrine), dopaminergic agents (e.g. dopamine, dobutamine) is contraindicated along with Linezolid. Linezolid can be taken without regard of food. The patient should be advised to avoid foods or beverages with high tyramine content while taking Zyvox. Zyvox should not be used in patients who are on monoamine oxidase inhibitors or within 2 weeks of discontinuing MAO inhibitors.
The recommended starting dosage of Linezolid is 600mg twice daily for 10-14 days.
